Community Health Worker – Community Connect

Community Health Worker (CHW) serves as a liaison between the community and healthcare/social service systems, addressing social determinants of health, promoting wellness, and facilitating access to medical, behavioral, and social services. Responsibilities include conducting outreach to underserved areas, performing in-person client visits, building trust through regular communication, assessing health and social needs, developing personalized care plans, providing health education on chronic disease management, preventive care, nutrition, mental health, and wellness, connecting clients to services (housing, food access, transportation, behavioral health), navigating health benefits, advocating to reduce barriers to care, documenting interactions in HIPAA-compliant systems, collaborating with clinical and social service partners, participating in meetings and trainings, and serving as a cultural broker to ensure culturally and linguistically appropriate services. The role requires frequent travel by vehicle within designated service areas and adherence to privacy requirements.
