Community Health Worker

Job Summary
Conduct daily outreach for health initiatives, advising individuals on diabetes, obesity, prevention, immunization, and childcare. Identify and contact high-risk groups including minorities, low-income residents, and pregnant women to provide one-on-one coaching and cultural mediation. Distribute informational documents, attend community meetings and health fairs to build relationships, and advocate for community health needs with government agencies. Refer members to essential health services and ensure parental understanding of childhood immunization access. Occasional weekends required.
Required Qualifications
- High School Diploma/GED
- Experience working in the community
- Experience with Community Organizations and Home Health follow-up
- Ability to work independently, set priorities and manage work within timeframes, read and analyze technical material, communicate effectively verbally and in writing, positive mental attitude
- Strong customer service orientation and the ability to handle highly confidential information essential
- Must be computer literate
- Will complete Community Health Worker (CHW) or Promotoras training
- Occasional Weekends

- Teaching experience
- Bilingual (English/Spanish)
