Job Summary
Conduct home and facility visits in Bennettsville, Lake View, and Marion to coordinate patient care, facilitate provider visits, and deliver medications and durable medical equipment. Perform routine venipuncture for laboratory testing, review medication instructions, and reconcile medication lists during these visits. Set up and instruct patients on Remote Patient Monitoring equipment, while coordinating telehealth appointments with specialists and pharmacists. Maintain accurate communication with the care team via phone and electronic platforms to ensure compliance with care plans. Requires reliable transportation, a valid driver's license, and ability to work 12-hour shifts from 7:00 AM to 7:00 PM.
Required Qualifications

- Service area in Bennettsville, Lake View, or Marion
- Certified Medical Assistant, Paramedic, or Licensed Nurse
- Certifications or licenses in good standing
- Phlebotomy experience
- Willingness to complete training and demonstrate competency in phlebotomy if not experienced
- Previous experience in public health or home health
- Experience with DME delivery or logistics operations
- Proficiency in using technology tools and platforms for remote patient monitoring, telemedicine, and electronic medical records
- Strong organizational and time management skills
- Ability to multitask and prioritize responsibilities effectively
- Ability to read and communicate effectively
- Strong written and verbal communication skills
- Basic computer knowledge
- Ability to work independently with minimal supervision and direction
- Good interpersonal and communication skills under all conditions and circumstances
- Ability to work flexible hours
- Ability to travel between offices, facilities, etc.
- Licensed driver with an automobile insured in accordance with state and/or organization requirements and in good working condition
- Clean driving record
- Experience operating vehicles in a professional capacity
