Community Health Worker (CHW) Supervisor

Job Summary

Conduct direct supervision of non-clinical Community Health Workers through regular check-ins, field observations, and performance evaluations. Coordinate monthly communications with CCPN Practices and PRRs while overseeing recruitment, onboarding, and retention efforts for the team. Manage workflows, processes, and programs to ensure staff alignment with organizational goals, and facilitate training on core competencies including cultural humility and soft skills. Monitor individual performance metrics, conduct chart audits for documentation accuracy, and track key performance indicators for member engagement and outcomes. Prepare reports for leadership, approve time sheets and reimbursements, and utilize data to drive performance improvement strategies. Maintain team accountability for meeting deadlines and targets while fostering a collaborative, mission-driven culture.

Required Qualifications

  • High school diploma or GED
  • Valid driver's license
  • Current auto liability insurance
  • Associate or bachelor's degree in public health, social work, or related field
  • Minimum 5 years of experience as a Community Health Worker
  • Experience in a human services or health care setting
  • Demonstrated knowledge of or ability to work and/or within a targeted community
  • NC CHW Certification
  • Community health principles
  • Social determinants of health
  • Experience working in patient and clinical data systems
  • Knowledge of geographical community resources
  • Knowledge of state and federal benefits system
  • Sensitivity to diversity of cultures, language barriers, health literacy and educational levels
  • Knowledge of medical terminology
  • Ability to shift strategy or approach in response to the demands of a situation

  • Repetitive wrist motion and occasional lifting/carrying of up to 25 pounds
  • Ability to operate standard office equipment (computer, keyboard, phone) with or without assistive devices
  • Must be able to lift 50 lbs

Desired Qualifications

  • Associate or bachelor's degree in public health, social work, or related field
  • Minimum 5 years of experience as a Community Health Worker or experience in a human services or health care setting with demonstrated knowledge of or ability to work and/or within a targeted community
  • Bilingual
  • At least 3 years of leadership, supervisory, or team coordination experience
  • NC CHW Certification preferred but not required
  • Computer skills and experience to include MS Office software
  • Organizational and time management skills
  • Community trust-building and culturally responsive communication
  • Ability to shift strategies based on situational needs
  • Ability to build rapport across diverse populations
  • Ability to manage multiple priorities effectively
