Job Summary
Assist patients in navigating healthcare services, insurance coverage, and community resources while determining and issuing Sliding Fee Scale discounts and Presumptive Eligibility for Medicaid. Provide patient-centered education, address social determinants of health, and advocate for timely access to medical, dental, and behavioral services. Conduct home visits, community meetings, and phone follow-ups to ensure continuity of care, maintaining accurate documentation in the electronic health record and complying with HRSA and HIPAA regulations. Collaborate with registration, billing, and care teams to refer patients to housing, food, and transportation resources, meeting monthly productivity goals and KPIs.
Required Qualifications
- Ability to pass additional background screenings for work with youth and senior services
- Authorization or certification to determine Presumptive Eligibility for Medicaid, or ability to obtain within the required timeframe
- 1 year experience in healthcare setting
- BLS certification
- First Aid certificate
Desired Qualifications
- Associate degree
- Community Health Worker certification
- Valid Driver's License
- Previous experience in an FQHC, community clinic, or safety-net healthcare setting
- EPIC, electronic health record experience
- Knowledge of HRSA requirements, Sliding Fee Scale guidelines, and Medicaid programs
- Bilingual or multilingual skills
- Experience addressing social determinants of health
