Community Health Worker - Chuukese

Job Summary

Conduct case management, outreach, and health education while providing oral and written interpretation services in English and Chuukese. Assist patients with intake forms, insurance verification, entitlement referrals, and medication requests. Manage front desk operations including appointment scheduling, patient reminders, registration, and billing support. Document all contacts in patient charts and attend training for service improvement. Serve as an advocate for quality health and social services within the Patient Centered Health Care Home team. Fluency in English and Chuukese is required, along with basic computer skills and knowledge of local community resources.

Required Qualifications

  • Fluency in English and Chuukese
  • Ability to read, understand and translate technical/medical instructions furnished in written, oral or diagram form in both English and specified language
  • Basic knowledge of computer skills
  • High School Diploma or GED
  • Regular and reliable on-site attendance

Desired Qualifications

  • Knowledge of the community and community service resources especially in Kalihi Palama area and Honolulu Downtown area
  • Certification as a professional translator
  • Certification as a professional community health worker
  • Post-secondary education
  • One year of experience in providing bicultural and bilingual services in a community health center or similar setting
  • Formal training in medical terminology
  • Formal training as a community health worker
