Job Summary
Community Health Worker (CHW 1) serves as a liaison between health and social services and the community, conducting in-person assessments to understand member care needs, navigating healthcare and social service systems, coordinating access to basic needs, advocating for members with providers and community resources, and traveling up to 75% for field-based care. The role supports case management, addresses social determinants of health, promotes prevention and health education tailored to community needs, and requires independence with guided framework, field-based/home-office workstyle, and potential TB, language assessment, and driver requirements. Location-based travel within Michigan (Wayne or Macomb County) with remote/home-office work arrangements; includes mileage reimbursement and hybrid/home-office considerations; requires valid driver's license and adherence to HIPAA and patient-facing protocols. Training occurs with potential occasional office visits for meetings; salary range is $41,900–$56,600 annually, plus benefits.
Required Qualifications
- 2+ years of experience with community resources, health agencies, or social services
- Intermediate proficiency in Microsoft Office (Teams, Excel, PowerPoint, Outlook, Word)
- Exceptional communication and interpersonal skills
- Ability to manage multiple priorities in a fast-paced environment
- Respect for cultural and demographic diversity
- Strong written communication and advocacy for members at all care levels
- LPN (preferred)
- CHW training or certificate, or willingness to complete within one year
- Bachelor’s degree in social work or related field
- Familiarity with state Medicaid program guidelines
- Experience engaging with Medicaid enrollees (physical and behavioral health needs, varied health literacy)
- Bilingual or Multilingual: English/Spanish, English/Arabic, or Chaldean Neo-Aramaic
