Job Summary
Community Health Worker (CDI) to support the CDC-funded Diabetes Initiative in Baltimore. Conduct outreach, screening, and referrals to diabetes prevention programs and essential resources; address social determinants of health; connect Baltimore City residents with education and care to prevent or manage type 2 diabetes. Responsibilities include attending partner meetings; achieving productivity goals; documenting assessments in EMR; enrolling uninsured clients into Medicaid programs; coordinating care with a primary medical provider; up to 12-month follow-up; trauma-informed, client-centered services; motivational interviewing; education on Maryland Medicaid benefits; referring residents to Diabetes Prevention Programs and Social Needs resources; conducting social determinants of health screenings and health risk assessments; outreach via neighborhood canvassing, home visits, and community events; diabetes education and materials dissemination; timesheet submission and required trainings. Qualifications include HS diploma or GED with relevant experience; reliable transportation; 1 year CHW experience; CHW certification preferred; bilingual (Spanish) preferred; strong computer skills (Word, Excel, Outlook, SharePoint). Required Qualifications
- High School Diploma or GED with significant human services experience
- 1 Year experience working in a Community Health Worker (CHW) role and/or direct experience providing outreach and client services in Baltimore City in the areas of Medicaid System Navigation, Social Needs Assessments and Care Coordination
- Community Health Worker Certification preferred but not required
- Bilingual candidates strongly preferred (Spanish)
