Job Summary
Conduct home visits to complete non-medical surveys and ensure patients reside in safe environments. Travel to patients' homes as determined by need and schedule, implementing patient care activities both initially and in six-month intervals. Maintain exceptional time management skills to meet deadlines while managing organizational and communication requirements. Requires a valid driver's license and own vehicle for travel to Buffalo. Previous homecare experience helpful.
