Job Summary
CHW provides navigational, case management, and home/community-based services to community members. Responsibilities include recruiting patients through outreach, building trust to provide support and education, advising on healthcare services, conducting periodic health-behavior assessments, educating patients on self-management and the health care system, documenting work per clinic policies, collaborating in a multidisciplinary setting, serving as liaison with stakeholders, possibly transporting patients to appointments and homes, attending ongoing training, and maintaining privacy and confidentiality. Requires flexible scheduling (nights/weekends as required) and strong communication; bilingual English/Spanish preferred.
Required Qualifications
- Two (2) years of verifiable experience providing information, education, intervention and/or referral services to culturally diverse populations.
- Community Health Worker certification in Rhode Island preferred.
- Experience with motivational interviewing, advising/counseling clients, and/or participating in health promotion and health education activities.
- Excellent verbal and written communication skills.
- Excellent organization, thorough record-keeping, follow through, and ability to juggle multiple priorities in fast-paced environment with multiple collaborators.
- Prior experience using an electronic health record preferred.
- Bilingual English/Spanish preferred.
