Job Summary
Assist patients in accessing recommended cancer screenings, follow-up care, and a full range of medical, behavioral health, and community services. Assess patients for social drivers of health and advocate for access to transportation, insurance, technology, and appointment scheduling. Provide education and coaching to parents, escort patients to appointments, and help obtain medications. Screen for program eligibility, complete care plans with the clinical team, and document all work in the Compass Rose module of EPIC. Maintain caseload size and meet monthly outreach and engagement productivity requirements. Participate in required trainings, meetings, and supervision while utilizing web-based applications to address population management goals.
Required Qualifications
- High School Diploma/GED
- at least 1 year experience in a health service related field
- Bi-lingual- English/Spanish or Chinese, Arabic, Russian or Yiddish depending on targeted patient population
- Computer literate (Microsoft Office), telephone, fax machine, and photocopy machine skills
- effectively communicate with all levels of the organization
