Job Summary
Facilitate patient empowerment by enhancing communication and resolving barriers to care adherence. Engage high-risk patients through community outreach and serve as a liaison between health services and the community. Collaborate with primary care teams to develop care management strategies for chronic disease self-management. Maintain consistent follow-up via phone calls, home visits, and alternative settings to ensure ongoing support. Link patients with appropriate community resources and conduct public education initiatives regarding Qualified Health Plans. Process and coordinate outgoing referrals to specialty providers, verify clinical documentation, and track referral status from initiation through completion.
Required Qualifications
- High School Diploma or equivalent
- Bachelor's degree from an accredited college or university in a related field preferred
- Two (2) years of experience working in community health, human services outreach, and/or healthcare preferred
- Knowledge and competencies with harm reduction, motivational interviewing, and relevant treatment modalities
- Familiarity with the culture and values of the local community
- Strong knowledge of principles and processes for providing customer and personal services
- Excellent verbal and written communication skills
- Proficient in Microsoft Excel
- Ability to actively listen, communicate effectively, and build rapport with clients
- Location: Columbia, MO / Hannibal, MO
Desired Qualifications
- Bachelor's degree from an accredited college or university in a related field preferred
- Two (2) years of experience working in community health, human services outreach, and/or healthcare preferred
- Desirable experience includes customer service, patient/peer coaching, medical patient interaction, community relations, outreach, data entry, and public speaking
