Job Summary
CHW responsibilities include health-related education, accompanying patients to appointments, making referrals, scheduling intake with care teams, documenting progress, educating on benefits (e.g., Medicaid), assisting with applications, conducting home visits, and supporting patients toward self-sufficiency. Requires bilingual Cantonese/English, HIPAA compliance, and ability to travel throughout NYC; position is on-site at CAMBA/NYP Brooklyn Methodist Hospital with a split management model.
Required Qualifications
- Two (2) years of experience as a Community Health Worker or working within the Health Care field providing services such as patient outreach, recruitment and enrollment; patient education, and/or care management.
- Bi-lingual in English/Cantonese strongly preferred.
- High school diploma or G.E.D. preferred.
Additional Requirements
- Evenings and/or weekends may be required
- Ability to travel within the five boroughs of New York City.
