Job Summary

COMMUNITY HEALTH WORKER - BEHAVIORAL HEALTH role focused on engaging patients in the SUD program, building trusting relationships between patients and medical teams, conducting internal and community outreach to identify patients who may benefit from services, coordinating with medical, dental and behavioral staff, guiding patients through internal and external resources, and utilizing electronic health records to input data and coordinate services. The position supports patients in their recovery journey, may assist with intake and scheduling, and emphasizes collaboration within an interdisciplinary team to promote coordinated care and ongoing participation in treatment.

Required Qualifications

  • Certification or training as a Community Health Worker, peer advocate, or related paraprofessional role preferred; BA level candidate with knowledge of addiction may be considered in lieu of CSW certification
  • Previous experience in a healthcare, social services, or community-based setting preferred.
