Job Summary
Coordinate with primary care treatment teams and families to address social determinants of health and facilitate access to medical, social, and mental health resources. Build relationships with local community-based organizations and providers to support families in navigating healthcare systems, while providing health education on wellness behaviors. Assist patients with program and benefits enrollment, follow up on referrals to address access barriers, and drive a vehicle in compliance with fleet management policy. Work Monday through Friday, 40 hours per week, splitting time between Springdale and Rogers Harvey Clinic locations. Requires a Bachelor's degree or equivalent experience and an active Arkansas driver's license.
Required Qualifications
- Active Arkansas Drivers License
- Bachelor's degree (or related work experience equivalent)
- 1 year relevant work experience
- Bachelor's degree from four-year college or university
- Four years related experience and/or training
- Equivalent combination of education and experience
- Drivers License - Arkansas Department of Motor Vehicles
- Must have active Arkansas Drivers License
- Bachelor's degree (or related work experience equivalent) required
- 1 year relevant work experience required
- Must have active Arkansas Drivers License

- Coordinate with primary care treatment team and family to address social determinants of health
- Work closely with clinic Social Worker and/or Behavioral Health Clinician to reinforce treatment recommendations with patient in community settings and facilitate access to resources and referrals
- Build relationships with local CBOs, outpatient providers, acute or residential psychiatric treatment facilities to support families throughout the process of accessing appropriate medical, psychosocial, and mental health services
- Provide health education to patients and families on lifestyle and behaviors that promote wellness
- Coordinate with providers and payers to help patients access the correct level of care in order to avoid healthcare crises
- Assists patients with enrollment in programs and benefits for which they are eligible
- Follows-up on referrals, resources, and recommendations to help address access barriers as they arise
- In compliance with Arkansas Children's Fleet Management Policy drives an owned, leased, or personal vehicle frequently in the performance of essential job functions
