Job Summary
Conduct standardized Health-Related Social Needs (HRSN) screenings for food insecurity, housing instability, and safety, utilizing motivational interviewing to assess patient barriers. Facilitate referrals to internal hospital services and external community organizations, including SNAP and Medicaid enrollment, while managing closed-loop follow-up to ensure successful connections. Develop partnerships with local stakeholders, maintain a resource directory, and participate in community outreach initiatives to foster trust and increase awareness of available services. Document all interactions in the electronic health record and submit monthly reports to the Oklahoma Hospital Association. This role is embedded within the hospital/clinic setting, requiring collaboration with clinical and population health teams to address social drivers of health and advance equity.
Required Qualifications
- High school diploma or equivalent
- Current, valid driver's license
- Clean driving record
- Ability to lift 50 pounds from the floor to waist level
- Ability to handle, finger or feel objects, tools or controls
- Reach with hands and arms
- Communicate in English verbally and in writing
Desired Qualifications
- Associate's or bachelor's degree in public health, social work, or related field
- Experience working in community health, social services, or healthcare settings
- Strong knowledge of local community resources and social service systems
- Excellent communication, interpersonal, and organizational skills
