Community Health RN - Part Time

Community Health RN role providing individualized, outcome-oriented nursing care using the nursing process in ambulatory care settings. Responsibilities include medication administration, planning and documenting patient care, coordinating cross-continuum care, and communicating with patients, families, and health care teams to achieve desired outcomes. Requires ability to work autonomously and in teams, with emphasis on patient/family education, safety, and compliance with policies. Relevant skills include effective verbal and written communication, coordination with physicians and care managers, and proficiency with health IT tools (Outlook, Epic, HealthStream, Workday). Licensure as an RN, BLS certification, and ambulatory care nursing knowledge are required or preferred per role duties.
