Job Summary
Community Health RN - Part Time provides individualized, outcome-oriented nursing care using the nursing process. Responsibilities include medication administration, planning and documenting individualized care plans, patient/family-centered care, cross-continuum care coordination, and evaluating outcomes of interventions. The role emphasizes risk-reducing, cost-effective care, collaboration with physicians, care managers, health care teams, patients and families, and participation in quality improvement, education, and compliance activities. Requires RN license, BLS, ambulatory care proficiency, strong communication, teamwork, and the ability to work autonomously.
Required Qualifications
- RN - Registered Nurse
- BSN preferred
- BLS – Basic Life Support
- Graduate of an accredited School of Nursing (BSN preferred)
- Minimum Work Experience: Previous Ambulatory Care RN experience preferred
- Excellent verbal and written communication skills
- Demonstrated proficiency in ambulatory care nursing, knowledge, and skills
- Demonstrates flexibility and ability to adapt to changing priorities and regulations
- Basic computer skills required
- Knowledge of health system and community resources preferred
