Job Summary
Community Health RN in a part-time ambulatory care setting provides individualized nursing care using the nursing process, including assessment, planning, implementation, and outcome evaluation. The role emphasizes medication administration, plan-of-care development and documentation, patient/family-centered care, patient outreach, and collaboration with physicians, care managers, and other health professionals to achieve desired patient outcomes. The RN works autonomously and in teams, participates in quality improvement activities, and adheres to policies and procedures. Requires RN licensure (Massachusetts) and BLS; BSN preferred. Proficiency in ambulatory care nursing, strong communication skills, and ability to use computerized systems (Outlook, Epic, HealthStream, Workday) are essential. The position supports a culture of safety, patient experience, continuity of care across the continuum, and compliant care delivery in the ambulatory setting. The role is based in Kingston, MA, with responsibilities centered on outpatient nursing care and patient-family engagement.
Required Qualifications
- RN - Registered Nurse
- BLS – Basic Life Support
- Graduate of an accredited School of Nursing, BSN preferred
- Ambulatory Care RN experience preferred
- Excellent verbal and written communication skills
- Demonstrated proficiency in ambulatory care nursing, knowledge, and skills
- Basic computer skills required
- Knowledge of health system and community resources preferred
