Community Health Program Coordinator

Identify high-risk patients with poor health outcomes and manage program activities including referral coordination, eligibility checks, and monitoring Community Health Worker caseloads. Support data collection and analysis for overall program evaluation. Minimum of 3 years of experience with data collection, management, and analysis is required, along with 2 years extracting information and exporting data to other applications. Experience with the Epic EMR system is a plus. Compensation ranges from $25.00 to $29.32 hourly.
