Job Summary
Conduct advanced clinical home care services including patient assessment, medication management, and patient/family education. Assume ongoing, primary responsibility to coordinate, implement, and continually evaluate home care needs for patients recovering from illness, injury, or surgery. Review patient insurance information, coordinate activities for individual care plans, and ensure pertinent findings are shared with family and professional caregivers. Requires current NYS RN license and registration, with a Bachelor's degree preferred or Associate's degree required. Minimum of one year acute medical-surgical nursing experience is required, and Certified Home Health Agency (CHHA) experience is preferred.
