Job Summary
Assess patients, manage medications, and educate families while coordinating and evaluating ongoing home care needs. Review insurance information to coordinate individual care plans and share pertinent findings with family and professional caregivers. Provide advanced clinical services for patients recovering from illness, injury, or surgery in their familiar surroundings. Requires current NYS RN license, Bachelor's degree preferred, and minimum one year acute medical-surgical nursing experience. Certified Home Health Agency experience is preferred.
Required Qualifications
- Current NYS RN license and registration
- Bachelor's degree in nursing
- Associate's degree
- Minimum of one year acute medical-surgical nursing experience
Desired Qualifications
- Certified Home Health Agency (CHHA) experience
