Job Summary
Conduct initial nursing assessments, design care plans, and administer medications including parenteral therapy to clients in the home. Provide health teaching, counsel on emotional and spiritual needs, and ensure client safety while documenting findings in computerized systems. Collaborate with physicians, intake staff, and home health aides for case management and training. Participate in monthly quality assurance reviews, assist with regulatory documentation, and coordinate services with community agencies. Perform field orientation and additional duties as requested.
Required Qualifications
- Current RN or LPN licensure
- Valid driver's license
- Auto insurance
- Reliable transportation
