Community Health Nurse

Assess and evaluate the health care needs of patients and families, considering physiological, psychological, social, and environmental factors. Develop care plans based on patient needs, physician orders, and identified goals, then implement and revise them through regular assessment and observation. Initiate preventative, therapeutic, and rehabilitative procedures as prescribed, administer medications and treatments, and delegate responsibilities to ancillary personnel while ensuring quality care and compliance. Complete and submit all required documentation that is timely, accurate, and relevant. Requires a current NYS Registered Nurse License with at least one year of hospital or nursing home experience and one year in a Certified Home Health Agency.
