Job Summary
Community Health Nurse provides direct home-visit nursing care to referred individuals and families, conducts comprehensive nursing assessments, develops individualized care plans, follows physician standing orders and clinical protocols, coordinates psychosocial and chronic disease care within home and community settings, delivers patient-centered education, coordinates and manages care with the Care Coordination and Case Management teams, documents encounters in Electronic Health Records, and participates in quality assurance and program-policies improvement. Requires an Associate Degree in Nursing, AZ RN license, CPR/BLS/ACLS/PALS/NRP certifications (or obtain within 6 months), potential Navajo-English bilingual skills, valid driver’s license, and ability to travel for home visits; standard public health nursing knowledge and ability to work with multidisciplinary teams are emphasized.
Required Qualifications
- Associates degree in Nursing
- Licensed as Registered Nurse in the state of Arizona
- Current CPR, BLS, ACLS, PALS, and NRP certification or certification obtained within six months of hire
- Bilingual English and Navajo language preferred
- Valid unrestricted driver’s license
- Background investigation satisfactory for Child Care position
- Knowledge of electronic health records
- Ability to travel and make home visits including in inclement weather
- Community/public health nursing knowledge
- Nursing standards of care
- Ability to multitask and work in high-risk settings
- Strong communication and teamwork skills
- Ability to work with multidisciplinary teams
