Community Health Navigator II- Chula Vista, CA- Field Position

Conduct independent field-based case management for moderate-to-high acuity members, managing a caseload up to 50 people. Perform door knocks, telephone outreach, and in-person visits to enroll new members and complete initial and follow-up assessments including PHQ-9 and required reassessments. Develop individualized care plans, coordinate referrals to health plans and community resources, and document compliant case notes in Salesforce Health Cloud. Mentor newly hired staff, participate in interdisciplinary case conferences, and escalate complex member needs while maintaining strict adherence to CalAIM, DHCS, and health plan requirements. This role requires substantial travel and is non-clinical in nature, focusing on developing foundational care coordination skills for high-cost Medicaid members.
