Job Summary
Community Health Manager oversees chronic disease prevention and management programs, coordinates data management and quality improvement activities, supports enrollment and outreach, supervises Lifestyle Coaches, collaborates with community partners, and assists in budgeting, reporting, and program evaluation. Responsibilities include developing tracking tools, ensuring program fidelity, creating SOPs, and supporting marketing efforts for program success across diverse populations.
Required Qualifications
- Bachelor's degree or 6 years’ experience working in related field in lieu of education
- 2 years professional experience in program development and leadership
- Health Coach or Community Health Work certification preferred
- Experience in working with historically marginalized populations preferred
- Bilingual in English/Spanish preferred
- Lifestyle Coach credentialing or train to become a Lifestyle Coach within 12 months of hire date required
- Must have the ability to travel regularly and reliably throughout a three-county service area
