Job Summary
Provides home health nursing care under the direction of a Registered Nurse to Rochester Regional CHHA clients, performing routine nursing services, documentation, and patient education in a home-care setting. Participates in CHHA Service Education, rotates holiday/evening/weekend duties, and collaborates with RNs to revise care plans as patient needs change. Requires NY LPN licensure, BLS, and a NY driver’s license; 40 hours/week, days; localities include Rochester, NY.
Required Qualifications
- Graduate from an accredited School of Practical Nursing
- Current LPN licensure in New York
- Basic Life Support (BLS) Certification from the American Heart Association
- A valid New York State Driver’s License
- Ability to pass a driver’s license background check
