Job Summary
Manage Community Health Team to implement population health strategies through integrated workflows, including team huddles, workplans, and regular 1:1 check-ins. Develop and implement strategies addressing Community Health Needs Assessment and collaborate with community stakeholders to establish referral protocols. Manage community partnerships to increase collaboration between primary care and community-based organizations, championing collaborative initiatives and communicating programs to the community. Monitor and manage the Community Health Team budget, tracking expenses for training and travel, while co-developing strategies aligned with clinical and quality initiatives to leverage ACO and payor partnerships.
