Job Summary
Conduct in-person and telephonic assessments to understand enrollee care needs, preferences, and socioeconomic barriers while evaluating the home environment. Collaborate with case managers, peer support specialists, and SDOH coordinators to assist enrollees with scheduling visits, address appointment barriers, and navigate the healthcare system. Act as an enrollee advocate by accompanying clients to provider visits, monitoring care plan compliance, and providing motivational interviewing to support medication adherence. Deliver culturally appropriate health education, coach on self-management of chronic conditions, and build relationships with community resources to identify gaps and facilitate referrals.
Required Qualifications
- Minimum two (2) years prior experience working with community resources, community health agencies/social service agencies (Area Agency on Aging, DME providers, Meals on Wheels etc.)
- Intermediate working knowledge using Microsoft Office Programs
- Strong understanding and respect of all cultures and demographic diversity
- Strong advocate for members at all levels of care
- Must reside in Central or Northern Louisiana
- Must be able to work a 40 hour work week, Monday through Friday 8:00 AM to 5:00 PM Central Standard Time
- Must be screened for TB
Desired Qualifications
- Bachelor's Degree in Social Work or related field
- Community Health Worker training and/or certificate or willingness to complete within one (1) year
- Familiarity with state Medicaid program guidelines
- Experience engaging with Medicaid enrollees, including those with physical health and behavioral health needs and varied health literacy
- Fluent in Spanish, French Creole, or Vietnamese
