Job Summary
Create educational programming on nutrition, hygiene, and chronic disease prevention while coordinating community health fairs with vital screenings. Facilitate monthly stroke support sessions, lead diabetes and falls prevention curriculum groups, and manage registration, scheduling, and virtual logistics for these programs. Organize the annual Women's Heart Symposium and Tobacco Competency updates, recruiting guest speakers and overseeing presentation materials. Represent St. Francis Hospital at local senior centers and libraries, and actively participate in the Nursing Shared Governance Council and JCAHO Stroke Accreditation Committee. Gather public health data through surveys to advocate for improved community wellness policies.
Required Qualifications
- Must be certified by or obtain a certification from the Association of Diabetes Care & Education Specialists (ADCES/AADE) as a Lifestyle Coach within 6 months of hire
- Must hold an active or obtain a certification from American Lung Association (ALA) or American Heart Association (AHA) Smoking Cessation Facilitator Certification within 6 months of hire
- Must hold an active leader certification or obtain a certification from the Wisconsin Institute for Healthy Aging (WIHA) / Wisconsin Institute for Health Behavior within 6 months of hire
- NY RN License required
- Excellent oral and written communication skills required
