Job Summary
Screen patients for health-related social needs and connect them to resources for food, transportation, diapers, counseling, and financial aid. Assist patients in navigating the healthcare system by scheduling appointments in primary care, imaging, labs, and maternal-fetal medicine departments. Provide car seat safety services, including fitting newborns and children, and complete documentation in the electronic health record to support care plans and billing. Deliver culturally congruent, trauma-informed health education to patients and families while training new staff on role responsibilities and available resources. Collaborate with interdisciplinary care teams and community partners to meet patient goals and strengthen referral networks. Participate in quality improvement initiatives and data collection to ensure program goals are met.
Required Qualifications
- High School Diploma / GED
- Two (2) years relevant experience working with underserved populations with medical and social needs
- Experience working in medical settings and interacting collaboratively with healthcare teams
- Experience working with pregnant patients
- Certification as Traditional Healthcare Worker (THW), Personal Health Navigator (PHN) or Community Health Worker (CHW)
- Certification as a child passenger safety technician (CPST) within six (6) months of hire
- Certified as a Spanish Interpreter within one (1) year of hire
- Experience with computer systems, including web-based applications and some Microsoft Office applications (Outlook, Word, Excel, PowerPoint)
- Basic electronic Health Records (EHR) Systems
- Basic medical terminology, healthcare and billing procedures, HIPAA regulations and confidentiality
- Maternal child and family programs at the county and state level
- Bilingual fluency in written and spoken English AND Spanish
- Effective verbal and written communication, active listening and interpersonal skills
- Excellent customer service skills including patience, empathy, problem-solving, and advocacy skills
- Time management, prioritization, attention to detail, and multitasking
- Patient support/coping during the labor and post-partum process
- Ability to prioritize workload, handle multiple tasks and work independently with limited supervision
- Demonstrated ability to work or interact effectively across cultures in a way that acknowledges and respects the culture of other individuals
- Demonstrated ability to work collaboratively in small teams to improve the operations of immediate work group by offering ideas, identifying issues, and respecting team members
- Use motivational interviewing and trauma-informed techniques
- Lead system improvements and advocate for change at a county and state level
Desired Qualifications
- Some College
- Three or more years of relevant work experience in mission driven organization with one of those years being work experience with underserved populations with complex medical and social needs
- Experience in community outreach and education
