Job Summary
Community Health Advocate role focused on one-on-one work with high-risk patients to improve health outcomes through home visits, needs assessments, resource connections (e.g., food delivery, social services), and collaboration with multidisciplinary care teams. Responsibilities include managing caseloads, meeting patients in homes/hospitals/clinics, conducting open-ended needs assessments, using motivational interviewing to help create person-centered action plans, providing emotional and logistical support, coordinating with care teams, and co-facilitating patient support groups where applicable. Strong communication, organization, empathy, and problem-solving skills, plus willingness to travel for home visits, are essential.
