Job Summary
Community Health Advocate embedded in Judy Centers to locate pregnant and postpartum women using trauma-informed outreach, reconnect them to HCAM’s Care Coordination Program, and connect clients to prenatal and family services. Responsibilities include building trust with high-risk clients, partnering with Judy Center staff, establishing a referral system between Judy Centers and HCAM, conducting face-to-face outreach in the community or clients’ homes, coordinating with OB/pediatric/primary care and Medicaid/MCOs, documenting activities in EMR, maintaining outreach trackers, and meeting performance targets (85% success rate, 100% on-time case closures). Required qualifications include a High School Diploma or GED (Bachelor’s in a health-related field preferred) and at least two years of maternal/child health outreach experience; bilingual Spanish preferred. Strong communication, organizational, problem-solving, and computer literacy (MS Word, Excel, Outlook, SharePoint) are emphasized.

- At least two years of experience providing home/community outreach to the maternal and child health population in Baltimore City
Desired Qualifications
- Bachelor’s Degree in a health-related area preferred
- Certification in Community Health Work is a bonus
- Bilingual in Spanish is preferred but not required
