Job Summary
Provide person-centered habilitation services in homes and communities, guiding individuals with developmental disabilities toward independence and enhanced community integration. Document all services contemporaneously, complete required reports, and maintain accurate attendance records. Monitor safety, address concerns immediately, and provide supervision per individual Safeguards and Staff Action Plans. Support advocacy, health care management, and relationship building while communicating professionally with families and team members. Attend supervision, complete mandatory training, and adhere to all safety protocols and agency policies. Work 9 hours per week at $19.00/hr, with overtime as needed to maintain staffing ratios.
Required Qualifications
- High School Diploma or GED
- Ability to read, write, speak and understand English
- Ability to maintain confidentiality
- Ability to work independently and handle multiple tasks simultaneously
- Proficiency in the use of a personal computer and appropriate software and E-Mail system
- Ability to stoop, lift, kneel, push, pull, reach, stand, walk, hear, listen and communicate appropriately

- Ability to maintain composure under pressure
