Job Summary
Provide training and instruction to individuals with mental retardation and developmental disabilities in their homes and in the community, implementing the Community Habilitation Plan to achieve valued outcomes; document progress and keep timely records; ensure health, safety and welfare; communicate with supervisor and families; submit weekly verifications and bi-weekly timesheets; maintain flexibility to meet scheduling needs; requires knowledge of Home and Community Based Waiver and Person-Centered Planning; strong verbal and written communication; ability to establish relationships with consumers and families; physical ability to assist in transferring and providing support to adults.
Required Qualifications
- High School Diploma or GED
- AA degree preferred
