Community Engagement Assistant

Job Summary
Conduct face-to-face assessments, crisis interventions, and treatment planning for consumers discharged from involuntary commitments, ensuring contact within two working days and weekly follow-ups for the first month. Coordinate interdisciplinary team meetings, link clients with community resources, and maintain accurate Electronic Medical Record documentation including progress notes and transportation logs. Provide ongoing support to increase daily living skills, attend mandated trainings, and uphold safety standards while adhering to Valleys Standards of Conduct and compliance requirements.
Required Qualifications
- High School Diploma or Equivalent
- Six months to one-year experience with MI/MH population
- Ability to comply with all of Valleys Policies and procedures
- Ability to comply with Consumers Rights
- Ability to comply with Valleys and Division departmental safety procedures
- Ability to read, write, understand and speak the English language
- Valid Drivers License & ability to legally transport consumers
- Ability to obtain and maintain CPR/First Aid, and MANDT Certifications
Desired Qualifications
- Knowledge of community resources for behavioral health problems including mental illness, substance abuse and Intellectual/developmental disabilities
