Job Summary
Connect caregivers and individuals living with dementia to community resources and clinical support to help them remain safely at home. Promote dementia-friendly practices, deliver educational materials and memory screenings, and collaborate with healthcare providers to encourage early detection and individualized care plans. Identify funding opportunities, maintain secure client information, track outcomes, and assess community barriers to care while ensuring HIPAA compliance. Manage financial records according to state and county requirements. Report to the Deputy Director, Office of Healthy Aging, Tennessee Department of Health, serving Henry, Benton, and Decatur Counties with some travel across Tennessee.
Required Qualifications
- Graduation from an accredited college or university with a bachelor's degree in public health, nursing, social work, or other comparable discipline
- Experience equivalent to two years of full-time increasingly responsible professional public health, nursing, social work or another related field
- Licensed practical nurses with five years or more work experience working in a healthcare, home health, or community health setting
- Additional graduate coursework in public health, community organizing, social work, nursing, or other related fields
- Qualifying full-time experience in public health, nursing, social work, or other related fields
- Personal experience caring for those with dementia
- Personal experience working in a setting where they came in close contact with persons with dementia or caregivers
