Job Summary
Community Coordinator oversees groups of participants across work sites, volunteer sites, recreational outings, and community activities; develops and maintains diverse volunteer, vocational, and educational opportunities; helps develop engaging curriculum to meet program goals; implements independent living, employment, self-advocacy and soft skills in daily activities; transports individuals to activities and employment sites; prepares and assists with required assessments and reports; participates in training, ISP meetings, and data collection for ISP's, behavior plans, and progress notes; collaborates with other coordinators and the Program Coordinator to ensure quality service delivery and represents Triangle to participants, families, and community partners; participates in biweekly team meetings and monthly supervision with the Program Coordinator; adheres to policy and procedures and maintains professional conduct. Qualifications include a high school diploma or equivalent, some college or related experience preferred, and MA driver's license with clean driving record.
