Job Summary
Community Connector Practitioner focused on strengthening the confidence and capacity of families to increase connection and belonging through direct support, group work, community programs and collaborative planning with Restoring Families providers. Responsibilities include managing a full caseload of families across Bayside Peninsula using trauma-informed, strengths-based approaches; developing safety plans and connection plans; delivering targeted community-based programs; fostering warm referrals and networking to connect families with local resources; maintaining collaborative relationships with Restoring Families Practitioners and the CCC team; facilitating group activities and ensuring flexibility to meet family needs (including after-hours and occasional weekends); supporting organizational place-based activation and documentation through client records and team meetings.
Required Qualifications
- Bachelor Level qualification in Social Work, Psychology, Social Sciences or equivalent.
- Experience supporting vulnerable members of the community to meet their goals using trauma informed, strengths based and person-centred approaches.
- Experience building strong relationships with key stakeholders, working as part of a multidisciplinary team to ensure wrap-around support to empower independence and connection to community level support.
- Strong understanding of the Child, Youth and Families Act 2005, Family Violence and Child Information Sharing Schemes and the MARAM framework.
