Job Summary
Community-based role coordinating and navigating services to reduce barriers and connect individuals with formal and informal resources. Responsibilities include assessing needs, maintaining ongoing contact with providers, advocating for clients, linking them to appropriate services, coordinating a network of community resources, coordinating referrals and wraparound service delivery, collaborating with partners for follow-up, and facilitating Resource Events while ensuring up-to-date information and referrals for the Community Resource Plan. Requires effective communication, autonomous work capability, and the ability to travel to community sites; minimum education includes a Bachelor’s degree in social services or behavioral health (or an Associate in Human Services with 2–4 years of experience), with bilingual capability preferred; must pass level II background screening and complete Community NAPPI training.
Required Qualifications
- Bachelor’s degree in social services or behavioral health field with at least one (1) year of experience working within a community setting or Associate in Human Services with 2-4 years of experience. Bilingual capability preferred.
