Job Summary
Community Connect Specialist responsible for making 50-60 in-person community visits per week (20-30 for part-time), building relationships with churches, senior ministries, civic groups, attorneys, food banks, public health agencies, and other partners, educating the community about Medicare guidelines and home health services, tailoring presentations and materials, participating in health fairs and events, researching new programs to improve service quality, and staying within a $500 monthly budget while completing weekly education plans and quarterly actions.
Required Qualifications
- Degree in marketing, health, or a related field OR licensure as a medical professional
- At least 1 year of experience in healthcare marketing, public relations, or clinical practice
- Knowledge of agency service lines and disease specialty programs
- Willingness to participate in community events and health fairs
