Job Summary
Provide evidence-based psychotherapy, conduct intake assessments, and develop treatment plans for adult clients, with a primary focus on older adults facing transportation or mobility barriers. Deliver services primarily through home visits and community-based locations while coordinating follow-up care, referrals, and discharge planning to ensure continuity. Complete clinical documentation, recordkeeping, and data collection in accordance with agency policy and state requirements. Collaborate with internal departments and external partners, including the Southwestern Vermont Council on Aging, to facilitate client access to community resources and supports. Participate in regular clinical supervision, team meetings, and community outreach activities as requested. Maintain professional standards of conduct, ethics, and confidentiality while providing education and training to staff.
Required Qualifications
- Masters Degree in Social Work, Clinical Mental Health Counseling, Counseling, Psychology, or a closely related field
- Licensed in the State of Vermont
- License Eligible (Rostered as a Non-Licensed Psychotherapist) in the State of Vermont
- Knowledge of the assessment and treatment of mental health, substance use, and co-occurring disorders
- Knowledge of and sensitivity to the unique behavioral health needs of older adults and geriatric populations
- Ability to provide recovery-oriented, person-centered counseling and support to clients and their families in community-based settings
- Ability to establish therapeutic relationships with clients from diverse backgrounds while maintaining appropriate professional boundaries
- Strong clinical assessment, treatment planning, and crisis intervention skills
- Excellent written and verbal communication skills, including timely and accurate clinical documentation
- Strong organizational and time management skills with the ability to prioritize competing responsibilities and manage an independent caseload
- Ability to work both independently and collaboratively as part of an interdisciplinary team
- Ability to provide high-quality, client-centered services in a fast-paced environment with limited resources
- Proficiency with electronic health records and standard computer applications
- Must have a valid driver's license, and reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont
- Successful completion of a criminal history background check after hire
