Job Summary
Provide individual treatment and supportive services to youth and families through role modeling, homework assistance, and participation in educational activities. Participate in developing individual service plans and conduct family team meetings to discuss ongoing decision-making processes. Function as an advocate by facilitating linkages to community resources and maintaining regular communication with guardians and foster parents. Assist with youth supervision and provide transportation as needed while collaborating with service providers like DSS and Mental Health Clinics. Document status and progress on all goals, including educational, vocational, and behavioral matters, completing all reports in a timely manner. Requires a flexible schedule including nights, weekends, holidays, and on-call hours, along with a valid driver's license and reliable vehicle.
Required Qualifications
- Minimum High School diploma
- Valid Driver's License
- Acceptable driving history to the Agency
- Reliable car
- Ability to work with economically and culturally diverse population
- Organizational skills
- Communication skills

- Must be able to work a flexible schedule including various hours, nights, weekends and holidays and on call hours
