Job Summary
Provide field-based care coordination and management to clients in Flagstaff/Coconino County, combining care coordination with supervisory responsibilities to enhance client outcomes through behavioral, physical, and social interventions. Develop and implement individualized care plans, connect clients with housing, food security, healthcare, transportation, and mental health resources, and advocate to ensure timely access to services. Conduct home visits, engage with community organizations and healthcare providers, maintain HIPAA-compliant EMR records, and track referrals and outcomes. Travel within assigned territory for training, meetings, and fieldwork; PRN scheduling with a minimum of one eight-hour shift per week and at least four shifts per month to remain active. Requires four to six years of related experience, Associate’s degree (or active LPN/RN/EMT/Paramedic), BLS, TB testing, and a valid driver’s license. Bilingual (Spanish/English) preferred.
Required Qualifications
- Associate’s degree in Social Work, Sociology, Psychology, Counseling, or a related field required
- Active LPN/RN licensure, EMT, or Paramedic certification; Bachelor’s degree preferred
- Current BLS certification and TB testing required
- Current and clear driver’s license
