Community Care Coordinator

Job Summary
Coordinate short-term care coordination for Medicaid recipients, performing home and telephone assessments to link clients with primary care, dental care, and behavioral health; lead outreach and collaboration with MCOs, providers, and community resources to reduce barriers to care; maintain monthly success and on-time case closure metrics; document outreach activities in the electronic medical database; require collaboration with OB, pediatric, and primary care services, as well as linkage to Maryland Medical Care Programs; ideal candidates hold a Bachelor’s degree and have at least 3 years of related experience, with strong communication and MS Office skills.
Required Qualifications
- Bachelor's degree in a related area; Human Services, Social Work, Health Education is preferred but not required
- Minimum of 3 years’ related experience
- Relevant clinical experience providing services to pregnant women and/or infants, preferably in a public health/community setting
- Knowledge, skills, and abilities in Community Outreach, Medicaid System Navigation, and working with HealthChoice Medicaid recipients and Managed Care Organizations
