Community Care Coordinator

Job Summary

Conduct short-term care coordination for individuals and families utilizing MD Medicaid by receiving referrals, performing home and telephone assessments, and partnering with Managed Care Organizations and providers to address barriers to care. Initiate client outreach within five to ten days of referral, connect recipients to medical homes and covered benefits, and triage clients to resources such as home visiting, WIC, and transportation. Maintain a monthly minimum success rate of 70% and on-time case closure rate of 90% while documenting all activities in the electronic medical database and preparing narrative reports. Collaborate with community agencies to resolve social determinants of health needs and host team meetings to discuss complex cases.

Required Qualifications

  • Bachelor's degree in a related area; Human Services, Social Work, Health Education
  • A minimum of 3 years' of related experience
  • Relevant clinical experience providing services to pregnant women and/or infants, preferably in a public health/community setting
  • Demonstrated knowledge in the following areas: Community Outreach and Medicaid System Navigation
  • Demonstrated knowledge and experience working with HealthChoice Medicaid recipients and Managed Care Organizations
  • Organizational and Problem-solving skills
  • Strong knowledge of community resources
  • Proven ability to work efficiently and meet deadlines
  • Organizational skills with the ability to meet a demanding workload
  • Excellent oral and written interpersonal and communication skills required
  • Strong computer literacy skills, specifically with MS Word, Excel, Outlook and SharePoint (365)
  • Adhere to all local, state and federal safety and environmental codes, ordinances, standards and laws
  • Adhere to all HCAM and local safety plans, policies, practices and standards
  • Be aware of and follow all safety rules of your work site
  • Report any unsafe conditions or accidents to your supervisor
  • Practice standard precautions (formerly universal precautions) at all times
  • Participate in mandatory or available safety training
  • Regularly required to talk, hear and respond to customers and employees
  • Frequently is required to stand; use hands to finger, handle, or feel; reach with hands and arms; climb or balance; and stoop, kneel, crouch, or crawl
  • Occasionally required to walk and sit
  • Regularly lift and/or move up to 25 pounds
  • Occasionally lift and/or move up to 50 pounds
  • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus

Desired Qualifications

  • Human Services, Social Work, Health Education, from an accredited college or university preferred but not required
  • Community Health Worker Certification a plus; the willingness to undergo certification training within 1 year of employment
