Job Summary
Community Care Coordinator/Account Executive who will transition patients to the homecare setting and collaborate with hospital facilities and physicians to coordinate care. Responsibilities include identifying future patients and determining home care eligibility, reviewing medical documentation and insurance, discharge planning, coordinating care across care teams, maintaining facility relationships, and ensuring compliance with Medicare/Federal guidelines. Must be an effective communicator with strong attention to detail, capable of building relationships with patients, facilities, and care teams. Preferred licensure in RN/LPN/PT/LSW and a valid driver’s license. This role emphasizes patient-centered care coordination and discharge planning within a home health and post-acute care context.
Required Qualifications
- Associates degree with a minimum of two years’ experience; or a combination thereof.
- Preferred Licensed in the state of services as a Registered Nurse (RN), Licensed Practical Nurse (LPN), Physical Therapist, Physical Therapist Assistant, or Licensed Social Worker (LSW).
- Valid driver's license and auto insurance in your name as a driver.
