Community Care Clinician

Provide supports and services to clients and families in natural environments to facilitate personal growth goals through individual and group therapeutic interventions. Assist with assessment and personal growth planning, including developing Wellness Recovery Action Plans, while linking clients to appropriate services, advocating on their behalf, and monitoring service effectiveness. Deliver assertive outreach, crisis services, and ongoing case management, including transportation assistance and acting as a representative payee when clinically indicated. Collaborate with primary care providers to address physical health conditions, promote disease self-management, and integrate natural community resources into the service delivery process. Document all outreach attempts and critical incidents, attend clinical staffings, and participate in quality assurance committees.
