Job Summary
Coordinate and manage community-based care for individuals within Albuquerque and Bernalillo County, performing Health Risk Assessments (HRAs), CNAs, and developing Comprehensive Care Plans (CCPs). Act as a liaison among health plans, providers, hospitals, and social services to ensure integrated, client-centered care. Travel throughout Albuquerque and surrounding areas to deliver ongoing care coordination, assess needs, inform members about community benefits, and ensure compliance with 42 C.F.R. § 438.208. Requires a Bachelor's degree (Master's degree preferred), CPR certification, and a valid driver’s license; prioritizes strong communication, collaboration, and ability to work with diverse resources. Two years of healthcare experience strongly preferred.
